The Functionality Chart:  This chart was inspired by an example I saw on the website www.momsinflight.bravepages.com.   The purpose of this chart is to keep my triple A personality in check and to communicate to those around me what my needs may be--I’m not so good at doing that on my own. I put a clothespin on the number that best represents where I’m at on any given day. My 7 year old reads it frequently when she gets home from school, so that she knows where things stand.  It’s really become a great tool for all of us and saves me from having to do so much explaining.  I have to admit though I have a very tough time sticking to that right column--sometimes its so much easier to just push on through, though you always pay later! (Case-in-point I would score at a level 4 today--and I’ve been on the computer for over an hour! Goodnight!)

The Wellness Scale:  I designed this to monitor my symptoms.  This is very helpful to measures changes before and after new treatments.  The name is sort of Orwellian in origin--I feel anything but well when I am watching my bodily functions so closely!  It is really helpful for patients and doctors alike to document these kinds of details consistently.
